The Stakes
Thailand has no public healthcare system that covers foreign residents. Every consultation, admission, scan, and procedure at a private hospital is billed in full at the point of care. For a retiree on a fixed income, a single major health event without insurance can be financially catastrophic.
Private hospitals in Thailand — which are where most English-speaking retirees will be treated — operate on a payment-first model. Without insurance, you pay before you leave. And for serious conditions, the bills are large.

Surgery, anaesthesia, intensive care, physiotherapy, and a typical 10–14 day inpatient stay are all included in that range. Without insurance, you pay every baht before discharge.
Premium Structure
Pacific Cross prices its plans using a combination of age bracket, plan tier, deductible level, and outpatient inclusion. Understanding these levers lets you build the most cost-efficient structure for your situation.
Premiums are set in five-year bands, not individual years. Rates recalculate when you move into the next bracket, not at every birthday. The tables in this guide reflect the 60–64 age bracket — the most common entry point for Thailand retirees.
Disclosed pre-existing conditions are excluded from coverage, not priced into the premium. You pay the same rate as a healthy applicant, but your known conditions are not covered. Non-disclosure can void your policy entirely.
The only factor that increases your premium is BMI. Applications with a BMI above 32.0 attract a loading; above 42.0, the application is declined. See the BMI loadings table below for the full scale.
What's Included
All Pacific Cross plans share a consistent set of core benefits. The tiers above Standard Extra differ in how generously those benefits are covered, not whether they are included.
Outpatient doctor consultations and prescription drugs are covered on plans that include OPD (Options 1 and 3, or as an add-on). Maternity, dental routine, and vision are not included in standard plans.

Body Mass Index is the only factor that increases your base premium. The table below shows the loading applied at each BMI band, using Maxima Inpatient Only with THB 40,000 deductible (THB 51,354 base) as the reference.

Most retirees in good health can self-fund outpatient visits (a typical GP consultation costs THB 500–1,500) and prefer to keep their premiums low. If your outpatient needs are high, Option 1 (full OPD included) makes more sense.
